Health Records

Health Services is also responsible for collecting and maintaining student immunization and other health records. Students are legally required to provide proof of immunization prior to attending the University.

Copies of health records can be provided to students who have graduated or are transferring. If you attended the Brooklyn campus over the past five years and need a copy of your immunization records, you may request an immunization transcript by completing the Authorization to Release Information. You may fax or email this form to us. We encourage you to call our office at  631.687.1261 to confirm that we have your immunization record on file prior to submitting your request.
